Providertech Presents Strategies for Scaling Population Health at Upcoming Conference
Providertech selected to speak at the Centricity Healthcare User Group conference

PHOENIX, Nov. 6, 2018 /PRNewswire/ -- Providertech, an industry leader helping healthcare payers and providers more efficiently manage clinical care, announced their speaking session in this year's Centricity Healthcare User Group (CHUG) Fall 2018 Annual Conference.

Held November 8th through 10th in Nashville, Tennessee, CHUG Fall 2018 is designed to help providers and care teams who use Centricity understand how to leverage data to successfully manage population health and mitigate risk under value-based care.

As a Centricity user, Providertech leverages technology to automate population health outreach, resulting in improved patient engagement, better clinical outcomes, and an enhanced patient experience. At CHUG Fall 2018, Providertech will present how they helped La Comunidad Hispana effectively scale population outreach without expanding their care teams and resources.

As part of the speaking lineup, Amy Bastianelli, LCSW, MPH, Director of Population Health at La Comunidad Hispana, will join Providertech's Vice President of Clinical Innovation, Lisa Blue, MHI, RN.

"As reimbursement models shift towards value-based payment, successful population health engagement requires new workflows aligned with value-based care," said Ms. Bastianelli. "When our communication workflows failed to translate into better care for our patients, we decided to try something new to reach patients how and when they wanted to be reached, without spending valuable staff time."

On Friday, November 9th from 11:00 a.m. to 12:00 p.m. at CHUG Fall 2018, Ms. Bastianelli and Ms. Blue will share:

  • How appointment reminder messaging decreased La Comunidad Hispana's no-show rate while improving provider satisfaction and schedule fill rate
  • Strategies for mitigating "phone tag"
  • How to scale outreach to hyper-targeted patient populations without scaling budget
  • Tips to automate workflows that align clinical measures and operations with technology

"We understand outdated care delivery models make it difficult to scale population engagement," said Ms. Blue. "By sharing how La Comunidad Hispana achieved a 75 percent decrease in time spent manually confirming appointments, we hope to help other healthcare organizations understand how to more effectively engage patients before, during, and after care using the resources they have today."
